Application Process:
1) Complete this application document, either on-line or download and print the file (see below).
2) Sign and return NACCFA’s Statement of Faith (see below). 
3) Determine your agency’s membership fee and attach your remittance to the application (below left).
4) For new members, please include the following documents with your membership application:
•Copy of agency license (or explanation if not licensed)
•Copy of Agency By-Laws
•Copy of Agency Mission Statement
•Copy of Agency 501(c)3 letter

Operating Budget of $2,000,000 and above: $750/year if paid by December 31, 2011; $800 if paid in 2012

Operating Budget of $500,000 - $2,000,000: $500/year if paid by December 31, 2011; $550 if paid in 2012

Operating Budget under $500,000:$200/year if paid by December 31, 2011; $250 if paid by 2012

Individual Membership* $100/year if paid by December 31, 2011; $125 if paid by 2012

*Individual membership is extended to those who subscribe to the Statement of Faith and Mission of NACCFA. These individuals cannot be employed nor affiliated with an organization that qualifies to be an agency of NACCFA. Individual membership dues afford all the benefits and conditions associated with NACCFA. Should the individual become employed or otherwise affiliated with an agency or an organization that qualifies for membership, that membership will be terminated with no refund of any remaining dues. (NACCFA By-Laws, Article III, Section 3.1, Paragraph 3)

STATEMENT OF FAITH

We believe the Bible to be the inspired, the only infallible, authoritative Word of God. 

We believe that there is one God, eternally existent in three persons: Father, Son and Holy Spirit. 

We believe in the deity of our Lord Jesus Christ, in His virgin birth, in His sinless life, in His miracles, in His vicarious and atoning death through His shed blood, in His bodily resurrection, in His ascension to the right hand of the Father, and in His personal return in power and glory. 

We believe that for the salvation of lost and sinful man, regeneration by the Holy Spirit is absolutely essential. 

We believe in the present ministry of the Holy Spirit by whose indwelling the Christian is enabled to live a Godly life. 

We believe in the resurrection of both the saved and the lost, they that are saved unto the resurrection of life and they that are lost unto the resurrection of damnation. 

We believe in the spiritual unity of believers in our Lord Jesus
